Do I actually need an invitation letter?

If you apply for a Russian tourist visa on a US, UK, Canadian or Australian passport — yes, it's a required part of the application. EU passports usually don't need one: they qualify for the $52 e-visa. If that's you, skip this and apply on the official e-visa site.

Is the invitation a real invitation from a person?

No. It's a formal confirmation document issued by a registered tour operator. Nobody is actually hosting you; it's a legal formality every tourist application needs.

Why the 3-year multi-entry?

For passports that qualify — US passports do — the consular fee is identical for a single-entry and a 3-year multi, so unless you're certain you'll never return, the multi is the better ask — with one catch: it can't be expedited, so if your flight is close, take the single.
